Step by Step procedure for issue of Death Certificate

The applicant will get the application form from the enquiry counter

The applicant will fill the application form and attach ID proof (Aadhaar cards) and go to the cash counter.

The official will check the particular in the computer system if the particulars are correct he will get the fees and issue the cash receipt to applicant with mentioning the date of collection of certificate
- If the particulars are not correct the official will inform to the applicant to contact the dealing hand for correction
- Correction in the Death Record

If the correction is minor regarding spelling mistake he will submit the application along with proofs and fees deposit on the cash window. Suppose the correction is major (change of deceased father, mother & address) the applicant will submit affidavit, statements of two credible persons, report from the institute & document proof. The applicant will deposit the fees on cash window. The dealing hand will issue the receipt to the applicant with mentioning the date of collection of certificate

2 Days | The Registrar approves the authenticity of the child name and other particular and return to the dealing hand |
1 Day | The dealing hand, hand over the application to the Data Entry operator for preparation of the certificate. The Data Entry operator prepares the certificate and hand over to the counter no. 1 or 2 for distribution |

2 Days | The Registrar approve the correction and return to the dealing Assistant for correction |
2 Days | The dealing Assistant corrects the record and put up the register for signature to the Registrar in record |
2 Days | The Registrar put his signature in birth & death record and returns to the dealing hand for preparing the birth & death certificate |
2 Days | The dealing hand, hand over the application to the Data Entry operator for preparing the births and deaths certificate. The Data Entry operator prepares the certificate and hand over to the counter no. 1 or 2 for distribution |
